Home Health Infusion Nurse Responsibilities

  • Administer infusion therapies including IV antibiotics, biologics, hydration, specialty medications, parenteral nutrition, and other prescribed treatments.
  • Perform comprehensive patient assessments before, during, and after infusion therapy, documenting findings and communicating changes to the care team.
  • Insert, access, and maintain vascular access devices such as peripheral IVs, PICC lines, midlines, and implanted ports.
  • Monitor for infusion reactions and intervene appropriately, following clinical protocols and escalation procedures.
  • Educate patients and caregivers on medication regimens, device care, symptom monitoring, and safety precautions.
  • Coordinate care with physicians, pharmacists, case managers, and other healthcare professionals to ensure continuity and accuracy of treatment plans.
  • Maintain accurate clinical documentation in accordance with regulatory, organizational, and payer requirements.
  • Manage supplies and equipment, ensuring proper storage, handling, and infection‑control practices.
  • Promote patient independence by reinforcing self‑care techniques when appropriate.
  • Respond to urgent patient needs and provide triage guidance within scope of practice.

Home Health Nurse Requirements

  • Registered Nurse with current unencumbered licensure to practice professional nursing in the state in which the care is being provided.
  • Minimum 1–2 years of recent clinical experience, preferably in infusion therapy, home health, emergency, ICU, or oncology.
  • Strong IV insertion and vascular access skills required.
  • Current BLS certification; ACLS preferred.
  • Experience with infusion pumps, specialty medications, and sterile technique.
  • Ability to work independently in the home environment with strong clinical judgment.
  • Reliable transportation and willingness to travel within assigned territory.
  • Excellent communication, documentation, and patient‑education skills.

Preferred Skills

  • ONS/ONCC chemotherapy certification (if applicable to role).
  • CRNI (Certified Registered Nurse Infusion) credential.
  • Experience with electronic medical records (EMR) in home‑based care.
  • Comfort managing complex or high‑acuity patients in the home setting.
