• Conduct comprehensive clinical assessments to evaluate patients' mental health status, utilizing DSM-5 diagnostic criteria.

• Develop individualized treatment plans that align with both the patient's clinical needs and personal goals, incorporating patient feedback in the process.

• Administer evidence-based therapeutic interventions tailored to patients' unique circumstances, including cognitive-behavioral therapy (CBT) and solution-focused therapy.

• Maintain accurate, up-to-date clinical records that comply with legal requirements and organizational protocols, ensuring confidentiality and thorough documentation of patient interactions.

• Collaborate actively with a multidisciplinary treatment team, which includes physicians, nurses, and other healthcare professionals, to coordinate and enhance patient care.

• Provide education and emotional support to patients and families, facilitating discussions that empower them to engage actively in their care and decision-making processes.

• Stay abreast of current research, methodologies, and developments in mental health, sharing knowledge within the team to optimize patient care strategies.

In this role, you will work with a diverse patient population experiencing a range of mental health challenges, particularly those navigating end-of-life issues and chronic illness. • A Master’s degree in Social Work from an accredited institution is required, ensuring a substantial foundation in clinical practice.

• A valid LCSW license in the state of Texas is mandatory to provide clinical services autonomously.

• Demonstrated knowledge of solution-focused psychotherapy and other evidence-based practices to inform your clinical approach.

• Exceptional communication skills, allowing for effective collaboration with colleagues, patients, and families, complemented by strong problem-solving abilities.

• Prior experience in hospice, palliative, or end-of-life care settings is highly desirable, contributing a refined understanding of the unique challenges and emotional dynamics present in this field.

• Experience with electronic health record (EHR) systems to ensure documentation is consistent and compliant with healthcare regulations.
