As an LCSW at our hospice center, you will be part of a collaborative and multidisciplinary team that comprises physicians, nurses, and other healthcare professionals. • Conduct comprehensive clinical assessments of patients to understand their mental health needs and develop individualized treatment plans that include both short-term and long-term goals.
• Employ evidence-based therapeutic interventions to address a range of mental health issues, adjusting strategies as necessary to ensure optimal patient outcomes.
• Maintain detailed and accurate clinical records promptly to facilitate communication within the multidisciplinary team and ensure compliance with regulatory standards.
• Collaborate consistently with our healthcare team to review and assess patient progress and adjust treatment plans based on collected data and feedback.
• Educate patients and their families on mental health practices, coping strategies, and resources available to them, while providing emotional support during difficult times.
• Stay informed on the latest research and developments in the fields of mental health and hospice care, incorporating evidence-based practices into your work.

In this role, you will work with a diverse patient population at various stages of life, including elderly patients facing terminal illnesses and their families who are navigating grief and loss. Understanding the unique emotional and psychological challenges these patients may face is crucial to providing empathetic and effective care.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of Texas is mandatory.
• Proven knowledge of solution-based psychotherapy interventions and methodologies.
• Exceptional communication skills both verbal and written, as well as strong problem-solving abilities.
• Ability to work independently and as part of a collaborative team.

• Prior experience in a hospice, palliative care, or healthcare setting is highly preferred.
• Experience in providing grief counseling or support is a significant advantage.
