• Conduct thorough assessments of patients’ needs and create individualized care plans that align with their goals and family preferences.
• Utilize evidence-based therapeutic techniques to provide counseling to patients and families, helping them navigate the emotional complexities of hospice care.
• Maintain comprehensive and confidential patient records, documenting all interactions, progress, and adjustments to the care plan in accordance with HIPAA regulations.

• Stay informed on the latest research and best practices in hospice and palliative care, incorporating new strategies into your work to enhance patient outcomes.

Your work will primarily serve patients in the hospice setting, many of whom may be dealing with terminal illnesses, chronic pain, or significant emotional distress related to their health challenges. You will work closely with their families, offering guidance and support in coping with the realities of end-of-life care, ensuring that emotional needs are met and informed decisions are made together.

• A Master's degree in Social Work from an accredited institution.
• An active LCSW license in the state of Tennessee.
• Demonstrated expertise in counseling techniques and crisis intervention.
• Strong communication skills, both written and verbal, to effectively engage with patients and team members.

• Experience in hospice, palliative care, or a related healthcare setting.
• Familiarity with case management practices and the psychological aspects of serious illness.
