• Conduct comprehensive initial assessments to understand the social, emotional, and economic aspects influencing patients and their families.
• Develop individualized treatment plans that are centered around client goals, ensuring alignment with their medical care and psychosocial needs.
• Implement evidence-based therapeutic interventions, such as crisis intervention and short-term solution-focused therapy, tailored to address trauma, anxiety, and depression.
• Document all activities, including assessments and treatment plans, in an accurate and confidential manner using our electronic health record (EHR) systems.
• Collaborate with physicians, nurses, and other healthcare professionals in interdisciplinary team meetings to discuss patient care strategies and progress.
• Utilize the latest research and best practices in social work to enhance service delivery and improve health outcomes.

This role is primarily focused on hospice patients dealing with terminal illnesses, along with their families. You will be working closely with diverse clients coping with a wide range of psychosocial issues, including end-of-life concerns, grief, and acceptance. Your holistic support will make a significant difference in their experiences and quality of life.

• A Master’s Degree in Social Work from an accredited program.
• An active LCSW (Licensed Clinical Social Worker) license in the state of Texas.
• Strong verbal and written communication skills, with the ability to build rapport with clients and families.
• Proficiency in utilizing technology for remote service delivery and documentation.

• Experience working within hospice or palliative care settings.
• Specialized training or certification in trauma-informed care, grief counseling, or related fields.
• Prior experience collaborating with healthcare teams to deliver integrated care.
