This role operates primarily in a remote setting, with occasional visits to local schools or community centers to engage with clients directly. The work hours are typically Monday through Friday, from 8:00 AM to 4:30 PM, although flexibility may be required for evening or weekend appointments as determined by client availability. As a Licensed Clinical Social Worker, you will engage with clients through virtual platforms as well as face-to-face, ensuring that accessibility and continuous care are maintained.

As a Licensed Clinical Social Worker, you will be tasked with a variety of responsibilities, including but not limited to:

• Conducting comprehensive psychosocial assessments to evaluate clients' emotional, mental, and social needs.
• Developing individualized care plans in collaboration with clients and their families, aligned with their personal goals and preferences.
• Providing direct counseling services that help clients navigate challenges such as grief, loss, or life transitions.
• Advocating for clients' rights and guiding them in accessing necessary community resources, programs, and support groups.
• Facilitating interdisciplinary team meetings to improve care coordination with nurses, caregivers, and other healthcare support staff.
• Regularly visiting clients to monitor their well-being, safety, and living conditions, addressing any emerging concerns.
• Delivering educational resources to families regarding community support options to enhance social engagement for seniors.
• Maintaining meticulous documentation of client interactions, assessments, and care updates in electronic health records (EHR).

The primary focus of this position is to serve the senior community in Edinburg, Texas. You will work with a diverse population ranging from independent seniors to those requiring more intensive support. Understanding the unique physical and emotional challenges faced by older adults is essential, as each client presents different needs and circumstances. As you build rapport with clients, you will play a pivotal role in fostering a supportive environment that encourages active engagement and independence.

• A Master’s degree in Social Work from an accredited institution is required.
• A current and valid license as a Clinical Social Worker (LCSW) in Texas.
• Minimum of two years of professional experience in social work, with preference for backgrounds in community health or geriatric settings.
• In-depth knowledge of social work principles, ethical standards, and state regulations.
• Strong assessment, counseling, and communication skills, with an ability to connect empathetically with seniors and their families.
• Proficiency in using electronic health record (EHR) systems and relevant software applications to ensure accurate documentation.

Experience working with a diverse senior population, understanding their unique needs and challenges, will greatly enhance your effectiveness in this role. Familiarity with community resources specific to Edinburg or the surrounding areas, as well as experience collaborating with multidisciplinary care teams, is highly advantageous. Skills in conflict resolution and family mediation will also be valuable as you navigate complex family dynamics.
