Job description

This part-time, remote RN position manages hospice triage and supportive care calls. This position will respond to incoming calls from patient's families and relay this information to the nursing staff to meet patient needs.

- Triage patient calls Monday-Friday beginning at 4:00 pm through 8:00 am every other weekend and reports to the on-call nurse when call-back visits are warranted.

- Identify patient needs and escalates to the RN on-call accordingly.

- Work as part of a healthcare team to assess patient needs, plan and modify care, and implement interventions.

- Perform supportive care calls as part of a Suburban supportive care program.

- Maintain accurate, detailed report of calls received.

- Communicate identified needs and potential solutions to administrator/designee.

- Ensure regulatory compliance in record keeping.

- Must be punctual and timely with documentation.

- Other duties as assigned by administrator/designee.

- Support agency and corporate policies, goals and objectives.

- Promote the hospice philosophy.
