Mindful Transcendence Mental Healthcare is seeking an experienced and compassionate Licensed Clinical Social Worker (LCSW) to join our dedicated team in a full-time remote capacity. This vital role involves delivering high-quality clinical services to clients facing major mental health challenges within our Hospice Center. You will be instrumental in creating treatment plans, facilitating therapy sessions, and ensuring effective care management for individuals experiencing severe emotional disorders.

This position will be remote, allowing you to connect with clients from the comfort of your own home while actively contributing to a multi-disciplinary team via digital communication tools. You will collaborate closely with physicians, nurses, and other mental health specialists within our Hospice Center, fostering a culture of teamwork and shared purpose. • Conduct individual assessments and create integrated care plans tailored to each client’s unique needs, particularly for those experiencing symptoms of major mental illness.
• Provide individual, group, and family therapy sessions that focus on enhancing coping strategies and emotional resilience.
• Monitor client progress through regular follow-ups, adjusting treatment plans as necessary, and ensuring that clients’ care is both relevant and timely.
• Actively participate in team meetings and case reviews, collaborating with other professionals to determine the best interventions and support services for clients.
• Conduct brief, evidence-based screenings to assess clients' needs, determining referral for further assessment when required.
• Ensure compliance with professional ethical guidelines and organizational codes of conduct while maintaining high standards of care.
• Assist with crisis intervention as needed, providing immediate support and direction to clients in distress.
• Engage in ongoing training and professional development to enhance clinical skills and knowledge in line with best practices.
• Complete detailed reports and documentation in a timely manner, ensuring accurate data is submitted for program evaluation and improvement.
• Serve on agency committees to enhance service delivery and maintain quality assurance in all clinical operations.

In this role, you will primarily work with individuals facing severe emotional and mental health disorders, many of whom are part of vulnerable populations. You will provide not only therapeutic support but also guidance through complex emotional journeys, especially in the context of hospice care, where the focus is on quality of life and dignity. Understanding cultural sensitivities and the diverse backgrounds of clients will be essential in tailoring your approaches to meet their needs effectively.

• Master’s Degree in Social Work (MSW) from an accredited program preferred.
• Current and valid Licensed Clinical Social Worker (LCSW) certification in the state of Florida.
• Minimum of 2 years of clinical experience, preferably in mental health settings or hospice care.
• Strong interpersonal skills, with a proven ability to establish rapport and trust with clients and team members.
• Ability to navigate and coordinate care effectively within various systems.

• Experience working in hospice or palliative care is highly desirable.
• Familiarity with integrated care models and methods for collaborating with interdisciplinary teams.
• Proficient in utilizing telehealth platforms and electronic health records management.
• Experience in crisis intervention and conflict resolution.

The salary for this position ranges from USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
