About the Position

NAVIX Diagnostix is seeking an experienced Registered Nurse to join our growing Clinical Operations team as an Assistant Remote Care Program Clinical Lead supporting our Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.

This is an exciting opportunity for a clinically strong RN who is passionate about patient care, enjoys working in a collaborative environment, and is ready to take the next step toward clinical leadership. In this role, you will partner closely with the Clinical Lead to support the delivery of clinically sound, CMS-compliant RPM and CCM programs across multiple healthcare organizations. You will play an integral role in day-to-day clinical operations, helping ensure patients receive timely, high-quality care while supporting program growth, quality initiatives, and operational excellence.

As an Assistant Clinical Lead, you will serve as a key clinical resource for monitoring staff, assist with patient alert management and escalations, support protocol implementation, and help drive consistency across multiple client programs. You’ll collaborate with physicians, nurses, medical assistants, and healthcare leaders to enhance patient engagement, improve outcomes, and expand access to care through innovative remote care solutions.

What You’ll Do

In this role, you will:

  • Monitor daily patient alerts, vital sign trends, and escalations to ensure timely and clinically appropriate patient care
  • Serve as a first-level clinical escalation resource for LPNs, Medical Assistants, and monitoring staff while supporting patient safety and quality outcomes
  • Support client onboarding, workflow execution, monitoring platform integration, and ongoing program operations across multiple healthcare organizations
  • Assist with the implementation and standardization of clinical protocols, escalation pathways, documentation workflows, and RPM/CCM program processes
  • Help ensure compliance with CMS requirements, documentation standards, quality initiatives, reporting activities, and continuous program improvement efforts
What You’ll Bring

Required Qualifications

  • Active Registered Nurse (RN) license in New York at the time of hire
  • Compact nursing license required
  • Bachelor’s degree in Nursing (BSN)
  • Minimum of 8 years of clinical nursing experience
  • Previous telehealth experience required
  • Experience with Remote Patient Monitoring (RPM), Chronic Care Management (CCM), care management, or remote monitoring programs
  • Familiarity with clinical documentation, EHR systems, healthcare regulations, and compliance requirements
  • Strong clinical assessment, triage, critical thinking, and communication skills

Preferred Qualifications

  • Clinical experience in emergency medicine, critical care, cardiology, primary care, or case management
  • Experience supporting clinical teams, staff education, workflow coordination, or leadership initiatives
  • Familiarity with value-based care models, chronic care management principles, and CMS program requirements
