This position offers a fully remote work setting, allowing flexibility in managing your schedule while delivering essential support to seniors at home or in community locations. Full-time employees are expected to work Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours as needed based on client schedules. As a Licensed Clinical Social Worker, you will:

• Conduct thorough psychosocial assessments of senior clients to identify emotional, mental, and social needs, ensuring a comprehensive understanding of their situation.
• Collaborate with clients and families to devise tailored care plans that reflect their specific preferences and goals, emphasizing empowerment and person-centered care.
• Provide counseling and emotional support to clients navigating complex life transitions, such as health challenges, bereavement, or changes in living arrangements.
• Advocate for clients' rights and access to essential community resources, actively working to eliminate barriers to care.
• Engage in teamwork alongside healthcare professionals—nurses, caregivers, and specialists—to streamline and enhance holistic care for our clients.
• Facilitate regular visits to monitor clients' well-being, evaluate their home environments, and promptly address any safety concerns.
• Encourage communication among family members involved in client care, helping to mediate conflicts and facilitate healthy discussions regarding care needs.
• Educate clients and families about available resources and support groups, actively promoting social engagement to improve overall quality of life.
• Maintain meticulous records of client assessments, care plans, and documentation, adhering to all ethical and regulatory standards in social work practice.
• Stay updated on current social work practices and policies to ensure adherence to the ethical codes of our profession.

The primary focus of this role will be on senior clients and their families, addressing the intricate challenges they face as they age. Our client population often includes individuals experiencing chronic illnesses, cognitive decline, and life-limiting conditions. Your expertise in social work will play a critical role in providing timely interventions that promote comfort and dignity in their end-of-life journeys.

• A Master’s degree in Social Work from an accredited academic institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• Minimum of two years of relevant social work experience, ideally within community centers or geriatric environments.
• Strong understanding of social work principles, particularly in aging-related issues and community resources.
• Exceptional assessment and counseling skills, with the ability to empathize with clients and families.
• Proficient communication and interpersonal skills to facilitate collaboration within interdisciplinary teams.
• Adept in utilizing electronic health record (EHR) systems for documentation and client management.

While not mandatory, experience in hospice care, home health, or geriatric social work is highly desirable and will be valued as part of your background. Familiarity with local community resources, networking abilities, and strong advocacy skills will enhance your effectiveness in this role.
