Overview:
Overview
We are seeking a compassionate and experienced Registered Nurse - Integrated Care Manager (ICM) for a temporary assignment (approximately 3 months, with potential for extension)
This position requires the ability to conduct face-to-face clinical assessments with members residing in Central Richmond of VA and the candidates must reside closer to this area.
The RN ICM works collaboratively with members, families, providers, and the care team to optimize health outcomes, coordinate services, and support members across the care continuum.
The RN will perform telephonic and in-person assessments, identify high-risk conditions, coordinate care plans, and assist with authorizations, referrals, and resource connections while ensuring compliance with regulatory and organizational standards.

Preferred
  • Bachelor of Science in Nursing (BSN)
Certification/Licensure
Required
  • RN license in the state of Virginia (or multistate compact license)

Experience
  • Experience in case management, care coordination, or utilization/care review
  • Experience conducting clinical assessments (telephonic and/or face-to-face)
  • Experience supporting members with chronic conditions, co-morbidities, disabilities, or complex care needs
  • Strong communication, clinical judgment, and problem-solving skills
