• Conduct comprehensive clinical assessments to understand the emotional and psychosocial needs of patients within our hospice program, and formulate tailored treatment plans that incorporate solution-focused interventions.
• Implement therapeutic modalities including cognitive-behavioral therapy (CBT), and other evidence-based practices to help clients cope with grief, loss, and mental health challenges associated with terminal illnesses.
• Maintain meticulous clinical records while adhering to HIPAA regulations. Your attention to detail ensures a high standard of confidentiality and professionalism.
• Actively collaborate with interdisciplinary care teams, including physicians, nurses, and chaplains, to create integrated care plans that address all facets of patient wellness.
• Assist families in navigating through difficult conversations about end-of-life care, offering emotional support and useful resources to empower them during the process.
• Engage in peer review and case consultations, consistently focusing on improving care quality and embracing learning opportunities in the ever-evolving mental health field.
• Keep current with the latest therapeutic techniques and research to continuously refine your practice and provide effective, evidence-based care.

You will be providing specialized care to patients nearing the end of life, many of whom may experience complex emotional reactions including anxiety, depression, and anticipatory grief. Your work will directly influence their experience, as well as that of their families, by fostering environments of compassion, support, and dignity during a most challenging time.

• Master's degree in Social Work from an accredited institution, demonstrating your commitment to academic rigor and advanced clinical training.
• Active and unrestricted Licensed Clinical Social Worker (LCSW) license in the state of Florida, ensuring compliance with professional standards and legal requirements.
• Proficiency in evidence-based treatment modalities and methodologies, particularly those pertinent to hospice and palliative care.
• Exceptional communication and interpersonal skills, enabling meaningful connections with patients and their families.
• Robust problem-solving skills and critical thinking abilities to navigate complex, emotional scenarios with grace and efficacy.
• A collaborative mindset within a team-oriented environment, valuing diverse insights and practices to enhance patient care.

Experience in hospice, palliative care, or similar settings will be highly valued, as you will encounter sensitive situations requiring a deft approach. Familiarity with community resources for terminal illness support will also enhance your effectiveness in this role.

• Annual salary ranging from USD 90,000 to 104,000, commensurate with experience and qualifications.
