• Conduct thorough assessments and diagnoses of patients, utilizing tools such as the Diagnostic and Statistical Manual of Mental Disorders to guide the formulation of individualized care plans addressing mental health needs.
• Continuously evaluate patient progress and modify treatment strategies to ensure effective support for their mental health challenges.
• Document all treatment records, reports, and clinical information accurately and in a timely manner, adhering to regulatory and departmental guidelines.
• Work collaboratively with patients, their families, and healthcare providers to design comprehensive treatment plans that align with patient goals and cultural considerations.
• Facilitate referrals and provide assistance in connecting patients with relevant treatment providers while nurturing productive partnerships with clinicians and referral agencies.
• Educate and mentor less experienced team members, including students and interns, in best practices for delivering mental health care.
• Participate in clinical supervision and peer review processes to support the professional growth of the team, including chart audits and collaborative case discussions.

This role primarily focuses on providing outpatient therapy for individuals and families affected by cancer diagnoses. • Master's degree in Social Work (MSW) from an accredited institution.

• Experience in developing and executing individualized treatment plans in collaboration with patients and families.
