Job description

PeaceHealth is seeking a RN Quality Coordinator for a Per Diem/Relief, 0.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $47.01 - $74.50. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.
This position is represented by a collective bargaining agreement. There may be more than one opening on this posting.
Job Summary
Responsible for the quality review of all OASIS/HIS admission documents and Plan of Care to ensure CMS program compliance is met. Works closely with the System Manager of Quality for Home & Community along with Home & Community leadership to promote regulatory compliance and excellent patient outcomes and reimbursement. Serves as a consultant, auditor and OASIS/HIS expert to Home Health staff across the PeaceHealth system.
Essential Functions
  • Ensures the timely review and processing of Home Health OASIS/HIS admission document and Plan of Care.
  • Serves as stakeholder in determining appropriate OASIS/HIS scoring for correct reimbursement.
  • Collaborates with leadership to audit and analyze performance trends in OASIS/HIS accuracy; assist in determining learning needs for clinical staff.
  • Educates and provides feedback to clinical staff to correct completion on OASIS/HIS admissions document.
  • Ensures the review of OASIS/HIS data items and ensures OASIS/HIS based process measures and OASIS/HIS based outcome measures to positively impact Home Health Five Star score and Hospice Quality ratings.
  • Ensures regulatory compliance with OASIS/HIS conventions and Plans of Care components.
  • Ensures timely processing of OASIS/HIS to meet CMS guidelines and CMS billing requirements.
  • Promotes evidence-based practice care and documentation of skilled care.
  • Reviews all assigned OASIS/HIS tasks on a daily basis.
  • Provides training to new employees for OASIS and HIS.
  • Joint visits with home and community caregivers in the home/clinical setting.
  • Provides clinical and quality leadership in collaboration with other OASIS/HIS reviewers.
  • In Home Health, the data sets reviewed by RN Quality Coordinators are the OASIS data sets that are specific to Home Health and not to Hospice.
  • Travel may be required.
  • Performs other duties as assigned.

Qualifications
Education
  • Accredited School of Nursing Required: Nursing

Experience
  • Minimum of 2 years Required: Nursing experience
  • Required: Experience in Home Health/Hospice
  • Preferred: Experience with EPIC EMR system

Credentials
  • Required: Registered Nurse in state of practice
  • Required: within 1 Year OASIS-D Quality Specialist

Skills
  • Excellent verbal and written communication skills including sensitivity to other cultures and ethnicities. (Required)
  • Strong analytical skills. (Required)
  • Previous training experience and curriculum design. (Preferred)
  • Must possess excellent ability for conceptual thinking, good listening, problem resolution and planning skills. (Required)
