A Hug Away Healthcare is a leading provider of home health and hospice services. We are seeking a compassionate and experienced Home Health and Hospice Nurse to join our team. This role involves providing high-quality care to patients in their homes and managing their cases effectively using the Kinnser EMR system.
Key Responsibilities:
  • Provide comprehensive nursing care to home health and hospice patients, including assessments, care planning, and implementation of care plans.
  • Utilize the Kinnser EMR system to document patient care accurately and efficiently.
  • Perform case management duties, including coordinating care with other healthcare providers, managing patient records, and ensuring continuity of care.
  • Educate patients and their families about their conditions, treatment plans, and available resources.
  • Monitor and evaluate patient progress, adjusting care plans as necessary.
  • Administer medications and treatments as prescribed by physicians.
  • Conduct regular home visits to assess patient needs and provide necessary care.
  • Communicate effectively with patients, families, and the healthcare team to ensure optimal patient outcomes.
  • Participate in interdisciplinary team meetings to discuss patient care and develop comprehensive care plans.

Qualifications:
