Remote RN Case Manager (Illinois RN License Required)

Are you a clinically experienced RN ready to transition into a meaningful, work-from-home role? Join our care management team as a Remote RN Case Manager, where you'll make a direct impact on member wellness through proactive, holistic care coordination.

What You'll Do

  • Assess, plan, and coordinate care for members telephonically and virtually
  • Develop proactive care plans to improve short- and long-term health outcomes
  • Use clinical judgment and data insights to address complex medical and social needs
  • Support smooth transitions across care programs and benefits
  • Collaborate with multidisciplinary teams to remove barriers and optimize care
  • Engage members through effective interviewing and education to support wellness

Schedule & Work Environment

  • Fully remote (no travel required)
  • Standard hours: Monday–Friday, 9:00 AM–5:30 PM (your local time)
  • Occasional evening, weekend, or holiday shifts as needed

Required Qualifications

  • Active Illinois RN license (no Illinois residency required)
  • 2+ years of RN clinical experience (all specialties considered)
  • Associate Degree in Nursing (ADN)
  • Willingness to obtain additional state licenses if selected (company-paid)

  • Certified Case Manager (CCM)

The typical pay range for this role is:

$66,575.00 - $142,576.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
