NOW HIRING: Telehealth Nurse Case Manager


Location: 100% Fully Remote (Work from Home)

Pay Rate: $35 per hour


Schedule

  • Full-Time, Monday-Friday
  • 8:00 AM-5:00 PM Eastern Time
  • No weekends or major schedule rotations


Overview

We're looking for an experienced Registered Nurse Case Manager to join our team in a fully remote role. If you have experience in case management working for a health insurance company and enjoy helping patients get the care and resources they need, we'd love to hear from you.

In this position, you'll speak with members over the phone, review their healthcare needs, and help coordinate the right services to improve their health and quality of life.


Job Duties:

  • Manage a caseload of health plan members by phone.
  • Complete health assessments and identify each member's care needs.
  • Develop individualized care plans and help members reach their health goals.
  • Coordinate care with physicians, specialists, hospitals, pharmacies, and other healthcare providers.
  • Educate members about their medical conditions, medications, and available community resources.
  • Help members understand their health insurance benefits and available programs.
  • Monitor member progress and update care plans as needed.
  • Accurately document all patient interactions in the electronic medical record.
  • Follow company policies along with state and federal healthcare guidelines.


Qualifications

  • Active Registered Nurse (RN) Compact License in good standing.
  • Minimum 1 year of recent managed care experience working for a health plan such as UnitedHealthcare, Blue Cross Blue Shield, Molina or similar companies.
  • Minimum 1 year of recent RN case management experience required.
  • CCM (Certified Case Manager) certification preferred.
  • Strong communication, organization, and computer skills.
  • Comfortable working independently in a remote environment.
