What is a remote RN case manager?

A Remote RN Case Manager is a registered nurse who coordinates patient care, manages treatment plans, and advocates for patients—working primarily from a remote location rather than in a traditional healthcare facility. They assess patient needs, communicate with healthcare providers, and help ensure that patients receive timely and appropriate care. Remote RN Case Managers often use technology to monitor patient progress, provide education, and facilitate communication between patients and the healthcare team. This role is crucial in improving patient outcomes, reducing hospital readmissions, and supporting overall healthcare efficiency.

What are the key skills and qualifications needed to thrive as a remote RN case manager?

To thrive as a Remote RN Case Manager, you need a current RN license, strong clinical assessment skills, and experience in case management or care coordination. Familiarity with case management software, telehealth platforms, and electronic health records (EHRs) is typically required. Excellent communication, critical thinking, and self-motivation are standout soft skills for this remote role. These skills ensure effective patient support, accurate care planning, and seamless collaboration with healthcare teams from a distance.

What are some common challenges faced by remote RN case managers, and how can they be addressed?

To address these, it is important to leverage secure telehealth platforms, establish regular check-ins with team members, and stay organized with digital case management tools. Continuous professional development in remote communication and time management can also help RN Case Managers thrive in a virtual work environment.

Job description

Overview
Shield Health is seeking motivated, patient-focused RNs, LPNs, and LVNs to join our Enrollment Team. This is a fully remote, full-time (but flexible!) nursing role focused on enrolling patients in our chronic care management program.

Please apply on our company site: https://recruiter.shieldhealth.net/survey/nurses

What You'll Do

As an Enrollment Nurse, your primary responsibility is enrolling patients in a chronic care management program. You’ll call eligible patients, educate them about the program, answer questions, and assist interested patients with the enrollment process.

Responsibilities

  • Make ~50 daily outbound calls to eligible Medicare patients
  • Educate patients about Shield Health's care management programs using a script to explain program benefits and services
  • Answer patient questions and address common concerns in a professional, patient-centered manner
  • Assist patients who voluntarily choose to enroll in completing the enrollment process
  • Accurately document patient interactions, call outcomes, and enrollment information
  • Meet weekly expectations for productivity, quality, and documentation accuracy

Our Perfect Candidate

  • Active RN, LPN, or LVN license
  • Strong communication skills
  • Excellent technical aptitude - can quickly learn our web-based software, EMRs and phone systems
  • Comfortable spending the majority of the workday on the phone
  • Enjoys working independently
  • Organized, reliable, and goal-oriented
  • Reliable internet connection and a quiet remote workspace

What Makes This Role Different

  • Fully remote
  • Real flexibility
  • Performance-focused environment
  • Opportunity for professional growth in a rapidly expanding healthcare organization

Schedule

Unlike many remote nursing roles, we do not require fixed shifts or rigid schedules. This is a full-time job, but with flexible, choose-your-own hours. Nurses can call patients between 8am and 6pm, Monday-Saturday. During our two-week orientation period, nurses need to be available regular business hours Monday-Friday. After that you can adopt a more flexible schedule that fits your life – i.e. four higher-output days, Saturdays, etc.

Compensation

Our productivity-based pay structure rewards efficient nurses with incentives and flexibility – you’re paid for your work, not your time. After training, nurses have the flexibility to choose the workload that aligns with your availability. Compensation reflects the workload you choose, and additional incentive opportunities may be available based on overall job performance and productivity. Training includes guaranteed compensation while completing onboarding and orientation requirements.

Benefits

  • Health insurance for eligible employees
  • Dental insurance for eligible employees
  • Vision insurance for eligible employees
  • Paid time off for eligible employees
  • 401k for eligible employees
  • Employee referral program
