Job Summary and Responsibilities

As the Director of the Complex Placement Team (CPT) you will be responsible for the overall operations of the CPT, day to day activities of the CPT team and their adherence to the Complex Placement Post-Discharge Financial Assistance policy and Complex Placement Playbook. You will ensure that care facilities contracted to provide post-discharge care utilizing CommonSpirit Health patient financial assistance meet standards of patient care, quality, safety, and fiduciary responsibility. You will also ensure that patients placed in their arrangements are case managed for transition to the next setting of care as soon as appropriate, and you will be responsible for ensuring that vendors under these arrangements include all necessary information requested regarding patient status and that they follow designated invoicing procedures.
To be successful in this role you will need extensive knowledge of healthcare operations, including patient care processes, care coordination across the continuum, strong communication and collaborative skills with varying healthcare providers and leaders. You will need to possess strong organization and time management skills, and be well versed in the technology and data analytics necessary for strategic growth to meet the needs of the region.

This is a remote position, with travel within California expected ~25% of the time.

Job Requirements

Required Education and Experience:

  • Bachelors Degree in Nursing (BSN) or Master's Degree in Social Work (MSW) required.
  • Minimum of five (5) years of clinical case management experience, or five (5) years of progressively responsible management experience required.
  • Extensive operational experience, program planning, implementation, staff development, and needs assessment in healthcare environment required.

  • Comprehensive knowledge of care coordination and discharge planning required upon hire.

Required Licensure:

  • California Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) required.
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM) required within twenty-four (24) months of start date.
