Job Description

Daily Responsibilities:  

This person will be working in the field directly with Members of the Medicaid population who has more than one chronic medical condition, and is at risk for more conditions. These members often have Primary Care Physicians and other Specialty Physicians to be cared for. These members are often substance abusers who have become paralyzed and need specialty care, for example.

This position includes going on a monthly basis to member's homes to assess care, quality of living. They will meet with the member in addition to follow up calls and arranging resource needs as they present.

Hours for this Position: Monday-Friday 8am-5pm

Qualifications

Bachelor's degree in social work with relevant experience

Experience with Health Home Case Management: F2F Community based Case Management in the Medicaid population

Seeking Experience in Medical/Behavioral social work

-Medical Social Work Preferred in hospital discharge planning

-Experience in Community based Case Management or Care Coordination (Discharge planning included in this) 

-Experience working with the Medicaid population who are at risk for one or more chronic condition

Computer Competency-MUST be skilled in Excel

Valid Driver's License and car as this position is remote and field

Must be able to handle at least 30 Case loads
