Regular business hours are maintained; however, some evening and weekend hours may be required based on client needs and program activities.

• Conduct comprehensive assessments to identify clients' unique social, emotional, and environmental challenges, utilizing standardized assessment tools.
• Develop personalized service plans in collaboration with clients, detailing measurable goals, targeted interventions, and progress monitoring.
• Provide individualized counseling and support focused on facilitating healing and connection for individuals and families navigating crises.
• Engage in crisis intervention, including risk assessments and the development of safety plans to protect clients in distress.
• Collaborate with external agencies and community organizations, ensuring that clients have access to necessary resources and services.
• Actively participate in case conferences and multidisciplinary team meetings to ensure cohesive care and strategize services effectively.
• Engage with the community through outreach initiatives, educating potential clients about available services and promoting advocacy.
• Maintain accurate and thorough documentation of client interactions and progress in accordance with ethical and regulatory guidelines.

This role involves working intimately with diverse client groups, particularly those facing terminal illness and end-of-life issues in our Hospice Center. Clients may include individuals with serious medical conditions, their families, and caregivers, all of whom require specialized emotional and psychological support during challenging times.

• A Master’s degree in Social Work from an accredited institution is required, ensuring foundational knowledge in social services methodologies.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is essential for this position.
• Demonstrated knowledge of social and economic justice issues that inform the counseling approach, combined with a commitment to trauma-informed care.
• Strong communication skills, particularly in cross-cultural contexts, to effectively engage with a diverse clientele.
• Robust problem-solving abilities that empower clients in crisis and facilitate the transition into supportive community resources.

• Experience in a social service agency or a related field is highly desirable to support effective team integration and collaboration.
• Familiarity with electronic case management systems and documentation practices to maintain comprehensive client records.
• Previous exposure to working within a Hospice setting or with terminally ill patients is a significant advantage.

The annual salary for this position ranges from $60,000 to $100,000, commensurate with experience and qualifications, along with a substantial sign-on bonus to support your transition to our agency.
