• Conduct thorough assessments of clients to identify their social, emotional, and environmental challenges, allowing for personalized support.
• Tailor individualized service plans that focus on specific goals, intervention strategies, and ongoing progress evaluation.
• Deliver psychotherapy and counseling to individuals and families facing various life challenges, enhancing their ability to cope with difficult situations.
• Engage collaboratively with clients to create service plans while effectively connecting them with relevant community resources.
• Provide crisis intervention services, conducting comprehensive risk assessments and developing safety plans as needed.
• Foster partnerships with community agencies and organizations to ensure coordinated services and strong advocacy for clients.
• Participate in community outreach initiatives to increase awareness of our agency's services and promote the social issues we address.
• Attend case conferences, team meetings, and ongoing training sessions to continuously enhance your skills and those of your colleagues.

Your primary focus will be on clients who are in the hospice care system, which often includes individuals facing terminal illnesses and their families. You will be instrumental in providing emotional support, guidance, and advocacy as they navigate complex end-of-life issues. Your work will play a critical role in enhancing the quality of life for these clients during their most vulnerable moments.

• Master's degree in Social Work from an accredited program is required to support your clinical practice.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory for this role.
• Strong foundational knowledge of mental health disciplines, social and economic justice issues, and trauma-informed care practices is essential.
• Outstanding communication skills, cultural competence, and effective problem-solving abilities to foster positive client relationships.

• Previous experience in a social service agency or a healthcare setting with a focus on hospice care is highly desirable.
• Hands-on familiarity with community resources and the ability to integrate them effectively into service planning is beneficial.
• Proven expertise in crisis intervention strategies and risk management will enhance your effectiveness in this role.

This position offers an annual salary ranging from USD 60,000 to 100,000, commensurate with experience and qualifications, accompanied by a sign-on bonus.
