In this remote role, you will work from the comfort of your own home, collaborating daily with a multidisciplinary team of professionals devoted to patient-centered care. Although the setting allows for flexible scheduling, you may be required to accommodate evenings and weekends to meet client needs effectively. • Conduct thorough psychosocial assessments to evaluate clients’ mental health needs, identifying strengths, resources, and areas requiring support.
• Deliver individual, family, and group therapy sessions utilizing evidence-based treatment modalities tailored to each client's unique circumstances.
• Implement a range of therapies such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care to meet the diverse needs of clients.
• Develop and document individualized treatment plans that outline specific therapeutic goals and interventions, continually adapting them based on client progress.
• Work closely with psychiatrists, psychologists, and other healthcare professionals to ensure an integrated approach to client care, addressing both mental and physical health concerns.
• Conduct risk assessments and safety planning, particularly in crisis situations, while providing crisis intervention when necessary.
• Maintain accurate and timely clinical documentation in accordance with ethical and legal standards, ensuring optimal compliance and accountability.
• Facilitate and lead group therapy sessions, psychoeducation classes, and peer support groups to enhance community support and client engagement.

Your work will primarily focus on clients within the hospice center, many of whom are navigating complex emotional and mental health challenges associated with life-limiting illnesses. You will provide essential emotional support and coping strategies to clients and their families, helping to foster resilience during some of life’s most challenging moments. This role demands not only clinical skills but also empathy, patience, and the ability to create a safe space for vulnerable populations.

• A master's degree in Social Work from an accredited program.
• An active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years’ experience in a mental health or outpatient setting, with a focus on hospice or palliative care preferred.
• Demonstrated proficiency in evidence-based therapeutic modalities and clinical practices.
• Strong interpersonal skills and effective communication abilities to foster positive interactions with clients and team members.

• Experience providing mental health services in hospice care or end-of-life settings.
• Familiarity with current industry best practices and evidence-based interventions tailored to support psychological and emotional well-being in terminally ill patients.
• A background in crisis intervention and risk assessment specific to vulnerable populations.
