• Deliver mental health support and counseling to patients and their families, addressing both immediate and long-term emotional needs.
• Coordinate care system processes aimed at preventing readmissions, participating in critical transition conferences with patients and families.
• Utilize advanced tools and technology to proactively identify and intervene with patients at risk of readmission.
• Ensure thorough clinical handoffs for at-risk patients, which may involve making referrals to appropriate services.
• Maintain detailed and accurate documentation of patient progress via electronic medical records, tracking the development of social work plans.
• Collaborate closely with an interdisciplinary team, effectively communicating to anticipate and meet patients' needs.
• Provide education to patients and families on transition plans and support available resources.
• Advocate for patients’ rights by ensuring access to protective services and supporting ethical decision-making processes.

• A master's or doctorate degree in clinical psychology, counseling, or clinical social work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited institution.
• An independent license in the state of residence (LCSW, LMFT, LPC) is mandatory.
• Demonstrated ability to manage a high volume of cases effectively, ensuring comprehensive care and communication with large healthcare providers and support staff.
• Proficiency in utilizing electronic medical records systems for documentation and communication purposes.
