This role operates in a fully remote setting, allowing for a flexible work-life balance while still being an integral part of the Peer Support Behavioral Center community. Counselors will have access to comprehensive electronic health record systems to streamline documentation and patient progress tracking.

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, contributing to an integrated healthcare team.
• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, and provide insightful recommendations to primary care providers regarding treatment goals and plans.
• Deliver brief, focused interventions for patients who are in immediate need of mental health support, ensuring prompt and effective service.
• Guide primary care providers in determining the most appropriate care setting for patients, facilitating referrals to specialty mental health facilities when necessary.
• Follow up with patients to monitor their progress and adjust treatment approaches based on their evolving needs.
• Develop relapse prevention plans where applicable, assisting patients in achieving stable mental health functioning.
• Identify at-risk patients and collaborate on plans to mitigate further deterioration of their mental health status.
• Coordinate the delivery of health services, ensuring continuity of care across various treatment providers, both within and external to the primary care setting.
• Maintain accurate documentation of patient diagnoses and progress in treatment charts, adhering to legal and ethical standards.
• Work closely with primary care providers to ensure seamless communication and effective collaboration in formulating and executing treatment plans.
• Empower patients with self-management skills and educational resources, enabling them to take an active role in their recovery journey.
• Provide consultation to clinic management and interdisciplinary team members regarding behavioral health services and program evaluation.
• Assist patients in adhering to medical treatments prescribed by primary care providers, offering coping strategies for managing treatment challenges.
• Adhere to the Code of Ethical Conduct for Clinical Professionals in all interactions.
• Perform additional duties as assigned to support the clinic’s mission and objectives.

In this role, you will engage with a diverse clientele comprising various age demographics and backgrounds, many of whom are grappling with mental health challenges exacerbated by social determinants of health. • A minimum of a Master’s Degree in Clinical Counseling or a related field.
• Possession of a current LPC (Licensed Professional Counselor) license to practice in Pennsylvania.
• In-depth knowledge of mental health assessments, treatment planning, developmental psychopathology, and various therapeutic approaches, including family therapy and group therapy.
• Strong understanding of the DSM-IV diagnostic criteria and associated behavioral therapy techniques.
• Excellent verbal and written communication skills; bi-lingual proficiency in English and Spanish is preferred.
• Strong mental and physical health to meet the demands of the position.
• Thorough awareness of the community resources available to support patients and their families.

• Experience in integrated healthcare settings, working closely with primary care teams, is highly desirable.
• Prior work with diverse populations, including underrepresented communities, will be beneficial.
• Experience in conducting mental health education workshops or group therapy sessions.

The salary range for this position is competitive, ranging from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
