We assure our clients that all services are delivered with complete confidentiality, and we accept Medicaid, Medicare, and most private health insurances.

Work Environment

This position offers the flexibility of remote work while maintaining strong connections with our community health clinic. Although you will primarily work from home, you will have access to ongoing training and support from colleagues and supervisors, and you will be part of a collaborative environment that emphasizes mental health and wellness. As an LPC in our clinic, you'll be an essential team member in a supportive setting that prioritizes continuous learning and professional development.

  • Facilitating referrals to specialty mental health facilities when necessary, while advising the primary care team on the best course of action for each client.
  • Initiating follow-ups to monitor client progress and adapt treatment plans as needed.
  • Developing relapse prevention strategies and supporting clients in maintaining stable functioning.
  • Detecting at-risk clients and crafting proactive plans to prevent escalations in their condition.
  • Coordinating behavioral health services and linking clients with external treatment providers, all while observing confidentiality protocols.
  • Documenting patient progress and maintaining accurate treatment charts.
  • Empowering clients with self-management skills and educational resources to foster their active participation in recovery.
  • Consulting with clinic management and team members to evaluate behavioral health outcomes and program efficacy.
  • Assisting clients in adhering to medical treatments prescribed by primary care providers, including coping strategies for medication side effects.
  • Upholding the ethical standards laid out by relevant professional organizations.
  • Performing additional duties as assigned to support clinic operations and client care.
Patient Population / Client Focus

Your work will focus on a diverse patient population, including individuals facing mental health challenges, families seeking support, and children in need of developmental guidance. By creating a safe and supportive therapeutic environment, you will foster strong relationships with clients, guiding them towards improved mental and emotional health outcomes.
