- Stay informed about the latest safeguarding policies and procedures, integrating them into daily service delivery while proactively addressing implementation challenges.
- Monitor outcome-focused practices, gather feedback from clients and families, and recommend adjustments to services to enhance overall effectiveness and satisfaction.
In this role, you will primarily serve clients facing psychosocial challenges associated with serious and terminal health conditions. Your work will involve addressing the unique emotional, social, and mental health needs of hospice patients and their families, ensuring they receive holistic support throughout their care journey. You will also assist those with substance use challenges, providing critical interventions and resources to help them navigate their recovery processes.
Qualifications- Education: A Master's Degree in Social Work from an accredited program is required for this role.
- Licensure: Must possess a valid and active Licensed Clinical Social Worker (LCSW) certification.
- Experience: A minimum of two years of experience working in a behavioral health setting, with a strong emphasis on both case management and direct patient care.
- Skills: Proficient in taking comprehensive psychosocial histories, conducting case management evaluations, performing mental health assessments, including suicide risk assessments, and utilizing effective interview techniques.
- Experience with hospice or palliative care environments is highly desirable, as well as familiarity with the unique challenges faced by clients in these settings.
