Job description

Position Overview

Resolute Healthcare Clinic is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic team. This full-time, remote position will involve working from a hospice center setting in Houston, Texas. The LCSW will play a crucial role in supporting individuals facing major mental illness and severe emotional disorders, providing essential services to enhance their quality of life.

About the Organization

At Resolute Healthcare Clinic, we are committed to promoting healthier communities by delivering high-quality, patient-centered healthcare services. Our focus is on reaching medically underserved areas, ensuring that all individuals receive appropriate care, regardless of their ability to pay. By fostering an inclusive environment, we strive to create a supportive network for both our patients and our employees.

Work Environment

This position is designed to be entirely remote, allowing the LCSW the flexibility to work from various locations while engaging with clients primarily through telehealth platforms and virtual meetings. Collaborating closely with an interdisciplinary team, the LCSW will engage with clients in their homes and communities, ensuring a holistic approach to care that respects individual backgrounds and experiences.

Key Responsibilities
  • Provide clinical assessments and support to clients experiencing mental health challenges, ensuring an empathetic and patient-centered approach.
  • Engage actively as a collaborative team member, participating in meetings and case discussions to achieve shared goals.
  • Facilitate and deliver therapeutic interventions, including individual, group, and family therapy, tailored to meet the unique needs of each client.
  • Conduct initial screenings and coordinate comprehensive assessments when necessary, employing evidence-based methodologies.
  • Create integrated care plans with actionable strategies, connecting clients to necessary services and resources.
  • Lead crisis intervention initiatives, remain available on-call as needed, and take decisive actions to ensure client safety and well-being.
  • Monitor client progress, documenting outcomes and adjusting treatment plans to optimize effectiveness based on evolving needs.
  • Participate in continuous professional development activities aimed at enhancing clinical skills and treatment effectiveness.
  • Contribute to quality improvement committees, sharing insights to enhance service delivery within the agency.
  • Ensure compliance with ethical guidelines and organizational standards, fulfilling administrative duties such as timely reporting and data submission.
Patient Population / Client Focus

The LCSW will primarily serve clients experiencing severe emotional disorders and major mental illness within the hospice setting. These individuals often require a compassionate approach, focusing not only on their immediate mental health needs but also on their overall quality of life, emotional support during transitions, and end-of-life care, should that be relevant.

Qualifications
  • A Master’s degree in Social Work (MSW) or a related field is required.
  • Current state licensure as a Licensed Clinical Social Worker (LCSW).
  • Strong knowledge of therapeutic practices, risk assessment, and crisis management techniques.
  • Exceptional verbal and written communication skills to effectively engage with clients, families, and interdisciplinary team members.
  • Adept at maintaining organizational skills and planning in a fast-paced healthcare environment.
  • High degree of cultural competency to work effectively with diverse populations.
Preferred Experience
  • A minimum of 2 years of experience as a clinical social worker is preferred, particularly in settings that provide mental health services.
  • Bilingual candidates, especially those fluent in English and Spanish, are strongly encouraged to apply, enhancing our ability to serve a broader community.
Compensation and Benefits

This role offers a competitive salary range of USD 80,000 – 100,000 annually, commensurate with experience and qualifications. In addition to a base salary, we provide an enticing sign-on bonus, comprehensive health, dental, and vision insurance, a 401(k) plan with company match, and generous paid time off policies. Employees also benefit from life insurance, profit-sharing opportunities, flexible spending accounts, tuition reimbursement, and referral bonuses.

Career Growth Opportunities

At Resolute Healthcare Clinic, we believe in fostering professional growth and development. The LCSW role offers various pathways for advancement within the organization, including leadership positions or specialized clinical roles. Employees are encouraged to engage in continuous learning opportunities to maximize their impact on client care and overall organizational objectives.
