Key Responsibilities
  • Conduct initial intake assessments tailored to each client's unique strengths and needs, ensuring that you gather comprehensive information to inform treatment planning.
  • Independently develop effective and individualized treatment plans that include specific, measurable goals and intervention strategies, while incorporating client feedback and preferences.
  • Provide therapeutic interventions to assist clients in achieving personal and interpersonal growth, employing a variety of evidence-based modalities as appropriate.
  • Regularly monitor and evaluate the effectiveness of therapeutic interventions and adjust treatment plans as needed to continuously improve client outcomes.
  • Document client progress with minimal required paperwork, including initial intake notes, treatment plans, and routine progress notes following each session.
  • Collaborate with interdisciplinary teams within the hospice center to ensure a comprehensive approach to care that addresses emotional, psychological, and social needs.
Patient Population / Client Focus

Your primary focus will be on clients who are navigating the emotional and psychological complexities associated with being in hospice care. You will support patients and their families in coping with grief, loss, and the myriad emotional responses that accompany end-of-life transitions. Qualifications
  • A Master’s Degree in Social Work or a related field is required.
  • An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory for this role.
  • A minimum of two years of clinical experience in mental health or a related field is crucial.
