Work Environment

This position is primarily remote, offering flexibility while ensuring that you can effectively serve our clients. Although most work is performed from your own home, you may need to make occasional visits to our community health clinic or client homes as required. Key Responsibilities

• Conduct thorough psychosocial assessments of seniors, evaluating their emotional, mental, and social conditions to identify their needs.
• Develop individualized care plans in collaboration with clients and their families, tailoring goals to align with their specific situations.
• Offer empathy and counseling, helping seniors navigate transitions such as relocation to assisted living, health challenges, or the loss of loved ones.
• Advocate for clients' rights, ensuring they have access to community resources and support networks, promoting their autonomy and dignity.
• Coordinate care with a multidisciplinary team of professionals, including healthcare providers, caregivers, and community organizations, to enhance client support.
• Regularly monitor clients’ well-being through virtual check-ins or in-home visits, assessing living conditions and meeting safety requirements.
• Act as a mediator to resolve conflicts among family members regarding care decisions, ensuring open communication and collaborative solutions.
• Educate clients and families about local resources, support groups, and programs aimed at fostering social engagement and improving quality of life.
• Maintain meticulous records of client interactions, assessments, and progress in accordance with ethical standards and organizational policies.
• Stay abreast of current social work practices, community options, and regulatory guidelines to ensure compliance and uphold our organization’s reputation for excellence.

Patient Population / Client Focus

This role primarily focuses on elderly clients and their families, addressing the complexities of aging with sensitivity and understanding. By offering tailored support, guidance, and resources, you will help empower seniors to maintain their independence, improve their mental health, and foster better family dynamics.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• Minimum of two years of relevant experience in a social work capacity, preferably within geriatric or community health settings.
• Comprehensive knowledge of social work principles, particularly in relation to aging, mental health, and available community resources.
• Exceptional assessment, counseling, and interpersonal skills, allowing for effective client engagement and collaboration.
• Strong communication abilities to effectively present information and facilitate discussions among interdisciplinary teams.
• Proficiency with Electronic Health Record (EHR) systems and various computer applications for documentation and case management purposes.

Preferred Experience

Experience in a community health setting or with a focus on elderly care will be highly valued. Familiarity with care coordination across multidisciplinary teams and experience using telehealth platforms for client interactions is also advantageous.

Compensation and Benefits
