Work Environment

This role will be conducted remotely, providing you with the flexibility to work from home while maintaining a structured schedule. Typical work hours will be Monday through Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours based on the needs of our clients. Given the nature of the senior care sector, flexibility is essential for timely interventions and support. You may also need to be on-call to address urgent matters that arise outside of standard hours.

Key Responsibilities

• Conduct comprehensive assessments to determine the psychosocial needs of seniors, taking into account their mental, emotional, and social circumstances.
• Collaborate closely with clients and their families to craft individualized care plans that align with their goals, preferences, and values.
• Provide emotional counseling and support to seniors, helping them navigate significant life transitions such as health changes, loss, and relocation.
• Serve as an advocate for clients, ensuring they receive the necessary resources and support from community services.
• Work collaboratively with an interdisciplinary team of healthcare providers, including nurses and caregivers, to deliver coordinated and comprehensive care.
• Perform regular home visits to monitor clients’ well-being, assess living conditions, and identify any safety concerns.
• Facilitate family meetings to resolve conflicts, improve communication, and engage family members in the care process.
• Educate clients and families about available community resources, support groups, and programs to enhance social engagement.
• Maintain meticulous records of client assessments, care plans, progress notes, and any significant interactions, ensuring compliance with ethical and legal standards.
• Stay abreast of changes in social work regulations and best practices to ensure the highest standards of care.

Patient Population / Client Focus

Your efforts will primarily focus on improving the lives of seniors, a population that faces unique challenges including physical ailments, social isolation, and mental health issues such as depression and anxiety. Qualifications

• Master's degree in Social Work from an accredited institution is mandatory.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW) is required.
• A minimum of two years of relevant experience in a social work setting, preferably focusing on geriatric populations or community centers.
• In-depth understanding of social work principles, specifically relating to aging and available community resources.
• Exceptional assessment, counseling, and advocacy skills.
• Ability to build rapport and show empathy towards seniors and their caregivers.

• Proficient in electronic health record (EHR) systems and basic computer applications.

Preferred Experience
