Your primary focus will be on senior clients, who face unique challenges as they navigate aging and the accompanying social and emotional hurdles. This position is pivotal in bridging gaps and facilitating communication among clients and their families, enhancing both awareness and access to resources within the community.

Qualifications
  • A Master's degree in Social Work from an accredited institution.
  • Current state licensure as a Licensed Clinical Social Worker (LCSW).
  • A minimum of two years of experience in a relevant social work role, preferably within community centers or settings that focus on elder care.
  • Knowledge of social work principles, aging-related issues, and local resources available to seniors.
  • Ability to demonstrate empathy, compassion, and effective communication skills tailored to vulnerable populations.
  • Proficiency in electronic health record systems and experience with common computer applications for documentation and reporting.
Preferred Experience
  • Experience working with interdisciplinary teams in a healthcare setting to coordinate care.
  • Familiarity with community outreach programs and the ability to connect clients with additional support services.
  • Exceptional problem-solving capabilities and strong interpersonal communication skills.
