Lakeshore Talent is in search of a FULLY REMOTE In-Home Services Coordinator for a 3-month contract opportunity supporting PACE participants in the Aurora, Colorado area.
 

Pay Rate: $20.67 - $24.81 per hour
Contract: at least 3 months, filling in for an LOA
 

Schedule Options:

  • Sunday – Thursday, 8:00 AM – 4:30 PM

  • Monday – Friday, 8:00 AM – 4:30 PM

Position Overview

The In-Home Services Coordinator is responsible for ensuring the effective delivery of in-home services for active PACE participants. This role oversees scheduling, service coordination, care plan implementation, provider orders, and staffing coverage while maintaining compliance with all applicable requirements. The ideal candidate will have strong scheduling and logistics experience, a healthcare background, and a passion for supporting seniors.

Required Qualifications
  • High School Diploma or GED
  • Minimum 2 years of related administrative experience
  • Strong scheduling experience, including geographic routing and logistics coordination
  • Valid state-issued driver's license
  • Reliable personal transportation
  • Good driving record and current auto insurance as required by law
  • Minimum 2 years of experience in gerontology and/or medical/social services
  • Minimum 1 year of scheduling or logistics management experience
  • Minimum 1 year of experience in a healthcare setting
  • Current knowledge of the physical and emotional needs of seniors who may be physically and/or cognitively impaired
  • Current First Aid and BLS Certification required prior to hire (American Heart Association or Red Cross certifications accepted)
Preferred Qualifications
  • Additional experience supporting senior populations
  • Healthcare scheduling and care coordination experience
  • Experience working in home health, community-based care, or similar healthcare environments
Key ResponsibilitiesClient Support & Service Coordination (50%)
  • Serve as liaison between participants, families, caregivers, and care teams
  • Ensure schedules align with care plans and provider orders
  • Coordinate services with community agencies and healthcare partners
  • Communicate schedule updates and changes to participants, caregivers, and staff
  • Monitor participant coverage and staffing needs
  • Investigate and report client care concerns and employee wellbeing issues
  • Conduct routine and unscheduled supervisory visits as assigned
  • Participate in daily team meetings and stand-ups
  • Maintain accurate and up-to-date participant schedules
  • Verify completion of scheduled visits and services daily
  • Maintain communication with field staff and leadership regarding scheduling needs and barriers
  • Participate in an on-call rotation
Administrative & Operational Support (50%)
  • Maintain and review participant records for regulatory compliance
  • Utilize scheduling and reporting systems to support operations and compliance
  • Monitor field staff performance and communicate concerns to leadership
  • Assist with caregiver development, training, and orientation
  • Schedule field staff according to care plans and provider requirements
  • Optimize schedules to minimize travel time, mileage expenses, and overtime
  • Document scheduling changes accurately in electronic records
  • Answer and route calls professionally
  • Support intake, referral, and inquiry processes as needed
  • Follow organizational policies and procedures
  • Assist with operational goals related to quality, census, and service delivery
  • Complete assigned standard operating procedures and administrative tasks
  • Provide backup support for leadership functions as assigned
  • Foster positive relationships and teamwork among staff
  • Attend meetings as needed, including care team and participant/family meetings
  • Provide direction and support to field staff regarding scheduling and logistics
