This role offers the flexibility of a remote work setting, enabling you to work from the comfort of your home while maintaining a connection to our hospice center team in Kansas City, Kansas. While the position predominantly involves remote work, some evening and weekend hours may be required to engage with patients effectively.

• Utilize your education and clinical training to conduct thorough assessments and formulate independent psychosocial diagnoses for patients, ensuring a personalized approach to mental health care.
• Develop and implement patient-centered treatment plans in partnership with clients, integrating their strengths and preferences into the therapeutic process.
• Employ evidence-based behavioral interventions, such as solution-focused treatment, motivational interviewing, and behavioral activation, to facilitate effective patient outcomes.
• Collaborate closely with a multidisciplinary team, including psychologists, primary care providers, and other clinicians, to deliver a holistic and integrated care experience for our patients.
• Monitor treatment progress through systematic tracking and assessment of patient responses, adjusting plans as necessary to meet their evolving needs.
• Assist with psychotropic medication management as prescribed, focusing on adherence, monitoring side effects, and evaluating overall treatment effectiveness.

Your work will primarily focus on patients receiving hospice care, often managing complex emotional and psychological challenges as they navigate critical life transitions. Building strong therapeutic relationships and providing compassionate support will be at the forefront of your role, ensuring patients and their families feel understood and cared for during this sensitive time.

• A Master’s degree in Social Work from an accredited institution is required.
• Possession of a current, valid, and unrestricted LCSW license to practice independently in mental health in Kansas is essential.
• At least 1 year of experience working as an independently licensed therapist is necessary to apply.

While not mandatory, experience in a hospice or palliative care setting is preferred. Familiarity with the specific emotional and psychological needs of terminally ill patients will enhance your effectiveness in this role.
