Documentation Review & ComplianceAudit patient charts (SOC, recertification, discharge, PRNs) for completeness, accuracy, and regulatory compliance.Verify that documentation supports homebound status, medical necessity, and skilled need.Review and correct OASIS data to ensure accurate PDGM grouping and reimbursement alignment.2. Axxess EMR UtilizationUse Axxess EMR to monitor documentation timeliness, track deficiencies, and generate QA reports.Identify and address documentation trends or concerns across disciplines.3.
