This role is remote, providing you the flexibility to work from the comfort of your own home while still being part of a close-knit team devoted to impactful social work. You will interact with colleagues and clients via digital communication platforms, ensuring seamless collaboration and support. • Conduct detailed evaluations and assessments to understand each child's unique needs and development trajectory.
• Participate in the development and implementation of Individualized Family Service Plans (IFSP), tailoring interventions to meet specific family dynamics and child development goals.
• Regularly monitor and evaluate outcomes, collaborating with a multidisciplinary team to ensure comprehensive service delivery.
• Provide essential support to families, offering guidance and resources that promote the emotional and developmental growth of their children.
• Ensure compliance with all relevant Federal and State regulations governing social work practice in hospice settings.
• Utilize evidence-based practices and adopt a coaching model to implement interventions in natural learning environments.
• Establish rapport with children and families to foster a supportive and nurturing therapeutic environment.

In this role, you will primarily work with children under three years of age who have disabilities or developmental delays. Children in our care are at a crucial developmental milestone, and your expertise will greatly aid them in acquiring physical, cognitive, social/emotional, and communication skills. Collaborating with families, you will focus on resilience and empower them through effective strategies tailored to each child’s needs.

• A Master’s Degree in Social Work or a Doctorate of Social Work.
• Active licensure from the Department of Public Health as a Licensed Clinical Social Worker (LCSW).
• A valid driver's license is required for travel to client homes or other meeting locations as necessary.
• A minimum of one year of experience in working with children, particularly in early intervention settings.

• Demonstrated commitment to utilizing Natural Learning Environment Practices in social work.
