This is a full-time remote position that allows for flexibility while emphasizing a collaborative atmosphere with a network of healthcare professionals, including Nurse Practitioners and Clinical Exercise Physiologists, who work hand-in-hand to deliver an exemplary patient experience. In your role as a Licensed Clinical Social Worker, your responsibilities will include:

• Collaborating with a dynamic team of healthcare professionals to design and implement our evidence-based virtual-first cardiac recovery program.
• Providing virtual psychotherapy sessions (45-50 minutes each) for a diverse panel of patients, helping them cope with the emotional challenges associated with their medical conditions.
• Evaluating patient symptoms, risk factors, and medical histories to develop accurate diagnoses and devise individualized treatment plans focused on holistic wellness.
• Utilizing clinical best practices and current research to substantiate treatment plans, ensuring they align with the latest standards of care.
• Regularly re-evaluating treatment plans as necessary, adjusting strategies to enhance patient safety and maximize therapeutic benefits.
• Encouraging patient engagement through effective counseling techniques, including mindfulness and motivational interviewing, to help patients achieve personal and program goals.
• Functioning autonomously to make clinical decisions based on professional judgment, ensuring adherence to regulatory standards.
• Documenting patient interactions and treatment progress comprehensively and timely to maintain accurate patient records and facilitate inter-professional communication.

This role will primarily focus on patients receiving hospice care who are managing serious, often terminal medical conditions. Many individuals in this setting face emotional and psychological distress due to their health circumstances. Your expertise will be critical in providing comfort, support, and treatment tailored to their specific needs, helping to bridge the gap between physical health and mental well-being.

The ideal candidate will possess:

• A Bachelor’s degree in Social Work, recognized in the state of New York, alongside active LCSW licensure.
• A passion for innovative care delivery and a commitment to redefining traditional approaches in cardiac and hospice recovery settings.
• Strong communication skills to effectively collaborate across professional disciplines, with an emphasis on compassion and understanding for patients.
• The ability to handle diverse patient needs and adjust clinical strategies promptly based on individual circumstances.

While not mandatory, the preferred candidate will have:

• Previous experience working within integrated clinical teams, particularly in outpatient behavioral health, cardiology, or hospice care.
