  5. Strong clinical support system including peer collaboration and guidance when needed
  6. Streamlined workflows and administrative support to allow focus on patient care
  7. Casual, professional work environment that values autonomy and work-life balance
  8. Opportunities for long-term career growth, leadership, and program development
  9. Mission-driven organization focused on improving behavioral health access

  • Address complexities related to aging, comorbid medical conditions, and polypharmacy
  • Maintain continuity of care through consistent patient engagement
  • Collaborate with clinical and administrative teams to support coordinated outpatient care
  • Provide education to patients and families regarding diagnoses, medications, and treatment plans
  • Maintain accurate, timely, and compliant documentation within the EMR

  • Active, unrestricted New York State Nurse Practitioner license
  • Ability to work full-time aligned with Eastern Time clinic hours
  • Experience treating adult populations with comfort managing geriatric patients
  • Strong understanding of how medical comorbidities impact psychiatric treatment planning
  • Prior outpatient or telehealth experience preferred
  • Ability to manage a consistent outpatient caseload independently
  • Strong clinical judgment and decision-making skills
  • Excellent communication and virtual patient engagement skills
  • Comfort with telehealth platforms and EMR systems
  • Spanish speaking preferred
