This role is fully remote, allowing for a balanced work-life integration while supporting clients across Augusta, Georgia, and beyond. You will be part of a collaborative team that ensures all staff are equipped with the necessary tools and support to deliver exceptional care. A robust electronic health record (EHR) system will assist in managing patient data and planning interventions effectively.

• Conduct thorough psychosocial assessments to evaluate patients' and families' emotional and mental health needs during end-of-life care.
• Implement a range of therapeutic interventions, including individual therapy, family therapy, and group sessions tailored to the hospice setting.
• Utilize evidence-based practices such as cognitive-behavioral therapy and grief counseling to address complex emotional issues.
• Develop and execute tailored treatment plans reflecting each client's unique circumstances, including their preferences and family dynamics.
• Collaborate within a multidisciplinary team, engaging with medical staff, nurses, and alternative care providers to ensure holistic care for patients.
• Provide crisis intervention and support during emergencies to ensure patient and family safety.
• Maintain precise and timely documentation, adhering to compliance standards and ethical guidelines, ensuring continuity of care.
• Facilitate psychoeducation and support groups for families to enhance their understanding and coping strategies through the hospice process.

In this role, you will work with diverse populations across various age groups who are confronting life-limiting illnesses. Your expertise will provide critical emotional support to patients facing complex issues related to grief, loss, depression, and anxiety. You will also work closely with family members, helping them cope with the dynamics of end-of-life care, which often involves profound emotional challenges and decision-making processes.

• A Master’s degree in Social Work from an accredited institution is required.
• An active state license as a Licensed Clinical Social Worker (LCSW) in Georgia.
• A minimum of two years of experience in a mental health setting, preferably within hospice or palliative care.
• Strong understanding of evidence-based treatment modalities relevant to hospice care, including grief counseling and trauma-informed practices.
• Excellent communication, interpersonal, and assessment abilities to engage effectively with patients and their families.

• Experience working in a hospice or end-of-life care setting is highly desirable.
• Familiarity with EHR systems and care coordination tools that ensure seamless collaboration and documentation.
• Knowledge of community resources and referrals available to support patients and their families during the hospice journey.
