This role is primarily conducted in a telehealth setting, allowing for a flexible work environment tailored to your personal and professional needs. Key Responsibilities
  • Conduct ongoing assessments to evaluate and meet the mental health needs of clients, utilizing evidence-based practices to tailor treatment plans accordingly.
  • Facilitate communication between various stakeholders, including supervisors, direct care staff, and family members, to ensure a comprehensive understanding of each client’s progress.
  • Develop and maintain a tentative scheduling plan for client sessions, home visits, and necessary observations, coordinating these efforts to track and support client growth.
  • Implement the program plan effectively by guiding and monitoring in-home observations, conducting chart reviews, participating in staff interviews, and performing data analysis.
  • Document ongoing treatment progress through detailed monthly and quarterly notes, guaranteeing that all information is available for future assessments and intervention strategies.
  • Actively participate in Interdisciplinary Team Meetings, sharing insights and collaborating with other healthcare professionals to enhance the overall care experience for clients.
  • Organize and lead training for staff members, ensuring that all team members are fully competent in implementing the program plan and maintaining accurate documentation.
  • Provide follow-up support through interviews, home visits, and observations, reinforcing the commitment to each client’s ongoing needs.
  • Serve as a liaison to the community and other mental health entities, advocating for client support and development goals.
  • Contribute to the development of training materials aimed at improving client programs and outcomes.
Patient Population / Client Focus

As a Licensed Clinical Professional Counselor at Motion Matrix Counseling, you will work with a broad range of clients, including both adolescents and adults facing various mental health challenges. This includes, but is not limited to, issues related to family dynamics, trauma, substance use, and overall emotional stability. The diverse demographic you serve will enrich your practice, allowing you to apply and expand your therapeutic skills in varying contexts.

Qualifications
  • A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
  • A current and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.
  • A minimum of two years of clinical counseling experience is necessary, ideally within a diverse setting.
  • Specialization in areas such as family therapy, couples therapy, substance use disorders, or trauma therapy is highly preferred.
  • A strong grasp of legal and ethical standards governing counseling practices.
  • Outstanding interpersonal and communication skills, with a proven ability to build rapport with clients from various backgrounds.
