Position Overview

Community Outreach Mental Care (COMC) is actively seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our remote team. As an integral part of our hospice center, the LCSW will provide high-quality mental health care to individuals experiencing severe emotional and mental health challenges. This full-time role is designed for a professional who thrives in a both collaborative and independent work environment, with a strong commitment to enhancing the well-being of underserved communities.

About the Organization

Community Outreach Mental Care is a leading healthcare provider in Hillsboro, Oregon, dedicated to forging healthier communities. Our mission focuses on delivering holistic, patient-centered services in medically underserved regions, ensuring accessibility regardless of a patient’s financial situation. We believe that high-quality healthcare should be available to all, and our daily work serves as a testament to our commitment to equitable, compassionate care. Our values and mission guide every project and initiative, as we strive to set an example for other health centers.

Work Environment

The LCSW position allows for a remote work setting, offering flexibility while maintaining a strong connection with clients and team members. The clinical team works collaboratively via virtual platforms, facilitating a cohesive approach to healthcare. Although the role entails working from home, our organizational culture emphasizes teamwork, communication, and empathy, ensuring that each team member is supported in their work and personal development.

Key Responsibilities

As a Licensed Clinical Social Worker, you will:

  • Engage directly with clients, providing therapeutic interventions both via telehealth and in community settings, focusing on those experiencing significant mental illnesses or severe emotional disturbances.
  • Develop tailored treatment plans in collaboration with multidisciplinary teams, creating integrated care plans that enhance access to necessary services and improving communication among clients, family members, and other providers.
  • Provide short, evidence-based screenings and assessments to identify client needs effectively, ensuring they receive appropriate follow-up care.
  • Lead individual, group, and family therapy sessions, adapting therapeutic techniques as per the unique needs of your client base.
  • Conduct crisis intervention when necessary, ensuring clients are safe and receiving the immediate care they require.
  • Participate in regular team meetings and committees aimed at improving our service delivery and maintaining high service quality standards.
  • Maintain accurate documentation, reports, and data submissions to comply with organizational guidelines and regulatory requirements.
  • Offer on-call support during specified hours, responding promptly to urgent client needs.
Patient Population / Client Focus

The clientele served by the COMC hospice center includes a diverse population, primarily individuals facing severe emotional challenges or mental health disorders. The LCSW will interact with clients from varied backgrounds and cultures, requiring sensitivity and adaptability in therapeutic approaches. A commitment to understanding and supporting underserved communities is vital, as many clients may come from low-income or marginalized demographics.

Qualifications

The ideal candidate must hold a Bachelor’s degree in Social Work or a related field and possess a valid Licensed Clinical Social Worker (LCSW) credential. Additionally, a minimum of two years of relevant clinical experience is required, demonstrating proficiency in mental health assessments, intervention strategies, and therapeutic practices. Candidates should also exhibit strong communication skills and cultural competence in working with diverse populations.

Preferred Experience

Preference will be given to applicants with experience in hospice, palliative care, or community mental health settings. Familiarity with trauma-informed care practices and crisis intervention techniques is highly desirable. Additionally, experience collaborating within multidisciplinary teams to create comprehensive care strategies will enhance your application.

Compensation and Benefits

This full-time position offers a competitive salary range of USD 80,000 to 100,000 annually, commensurate with experience. Employees are also entitled to a comprehensive benefits package that includes health insurance, flexible schedules, professional development opportunities, and access to mental health resources. We emphasize work-life balance and provide support for ongoing education and training initiatives.

Career Growth Opportunities

At Community Outreach Mental Care, we believe in nurturing talent and providing pathways for professional development. LCSWs have opportunities to advance into clinical supervisory roles, program management, and leadership positions within our organization. We support continuing education and encourage our team members to pursue further specialization based on individual interests and organizational needs.
