Key Responsibilities

As a Licensed Clinical Social Worker at Support Wellness Healthcare Services, your responsibilities will include:

  • Beginning discharge planning on the day of admission to enhance the quality of care and expedite the transition process.
  • Working collaboratively with multidisciplinary teams to ensure exceptional clinical standards are maintained.
  • Documenting interactions and maintaining accurate records in compliance with organizational policies, including initial assessments, treatment plans, and case notes.
  • Communicating effectively with the Utilization Review and Business Departments regarding authorization and billing processes.
