Community Care Program (CCP) Consultant

Position Overview

PurposeCare is seeking an experienced and strategic Community Care Program (CCP) Consultant to support the continued growth and development of our Illinois CCP service line. This is a 90-day consulting engagement focused on evaluating PurposeCare's current CCP footprint, identifying scalable growth opportunities, and developing a comprehensive operational and community partnership playbook to support long-term expansion.

The ideal consultant will bring deep knowledge of the Illinois CCP landscape, strong operational and relationship-building experience, and the ability to develop actionable strategies that can be executed by operational leadership teams.

This role will work closely with executive and operational leadership to analyze current workflows, identify opportunities for improvement, strengthen referral and community partnerships, and create a scalable framework for future growth.

Key Responsibilities

  • Conduct a comprehensive assessment of PurposeCare's current CCP operations, workflows, referral channels, and market footprint
  • Evaluate long-term growth opportunities and operational scalability within Illinois CCP markets
  • Develop a strategic playbook outlining recommendations for:
    • CCP growth strategy
    • Community partnership development
    • CCU relationship mapping
    • Outreach and referral generation activities
    • Operational best practices
  • Identify key operational gaps, barriers, and opportunities impacting census growth
  • Partner with leadership teams to align strategy, operational execution, and growth priorities
  • Assist in developing the ideal profile, expectations, and structure for future Outreach Coordinator roles
  • Provide recommendations around community engagement and referral-generating business activities
  • Collaborate with operational leaders to ensure recommendations are realistic, scalable, and execution-focused
  • Deliver final recommendations, tools, workflows, and implementation guidance at the conclusion of the engagement

Qualifications

  • Extensive knowledge of the Illinois Community Care Program (CCP)
  • Experience working within home care, healthcare operations, community outreach, or healthcare growth strategy
  • Strong understanding of CCU relationships, referral development, and Medicaid/community-based programs
  • Proven experience building, scaling, or optimizing operational programs
  • Strong analytical, organizational, and strategic planning skills
  • Ability to work collaboratively with executive and operational leadership teams
  • Exceptional communication and relationship-building abilities
  • Experience creating operational playbooks, workflows, or growth strategies preferred
