In this remote role, you'll be the bridge between enrollment and engagement-ensuring that families who've said "yes" to support are smoothly transitioned through the multiple steps of the GUIDE application process. This isn't just a job-it's a chance to help caregivers feel confident, informed, and supported as they begin a journey that could transform their caregiving experience.

Caregivers of people living with dementia often feel overwhelmed and under-supported. Ceresti's GUIDE program offers personalized coaching, care coordination, care planning, respite care, and resources-at no cost to eligible families.
As a Program Coordinator, you'll ensure that every patient and their caregiver moves through the onboarding process with clarity, care, and confidence. You'll monitor assessments, track progress, coordinate care and prepare families for their first connection with their dedicated coach.
What You'll Do
  • Guide patient and their caregivers through the onboarding process after enrollment
  • Monitor completion of dementia screening and health assessments
  • Coordinate tablet shipment and provide orientation to the GUIDE program
  • Schedule and track formal neurology evaluations with our clinical partners
  • Review documentation and prepare applications for CMS submission
  • Track progress and report status updates to internal teams
  • Ensure a warm handoff to Engagement Specialists and Coaches
  • Collaborate on software workflows
  • Adapt to evolving processes and contribute to continuous improvemen
