Key Responsibilities
  • Conduct comprehensive assessments to identify the unique needs of patients, ensuring a tailored approach to treatment that is informed by ongoing evaluations.
  • Maintain open lines of communication with supervisors, direct care team members, family members, and other relevant parties regarding each clientโ€™s progress and treatment plan.
  • Develop and share a structured schedule with supervisors for therapy sessions, home visits, and necessary observations to support case management.
  • Facilitate home visits and coordinate in-home observations, conducting thorough chart reviews and interviews to gather valuable insights and data.
  • Guarantee that implemented treatment plans are executed effectively and modify them as needed based on client feedback and outcomes.
  • Actively participate in Interdisciplinary Team Meetings to collaborate on treatment strategies and share insights that benefit client care.
  • Document monthly and quarterly progress notes to track client development and ensure continuity of care.
  • Organize training sessions for staff to ensure proper understanding of program plans and documentation requirements.
  • Engage in follow-up interviews, home visits, and observations to provide ongoing support and address the evolving needs of clients.
  • Maintain organized records that facilitate information sharing among relevant parties and support the coordination of care.
  • Serve as a liaison with the broader mental health community, advocating for client needs and helping to achieve developmental goals.
  • Contribute to the creation of training materials aimed at enhancing program delivery and improving client experience.
Patient Population / Client Focus

Your role will encompass working with a wide range of clients, including adults and youth facing various mental health challenges. You will utilize your expertise to provide counseling on issues such as family dynamics, trauma, and substance misuse, ensuring each patient receives the support they need in a compassionate and understanding environment.

Qualifications
  • Master's or Doctoral degree in Counseling, Psychology, or a related field.
  • Active and unrestricted licensure as a Professional Counselor in the state of Colorado.
