Key Responsibilities

  • Design and implement comprehensive care plans that facilitate access to an array of support services, creating a seamless continuum of care.
  • Maintain accurate documentation of client interactions, therapy sessions, and care plans to ensure compliance and facilitate continuity of care.
  • Participate in after-hours on-call coverage as necessary, providing additional support to clients in crisis.
  • Contribute to quality improvement initiatives by participating in agency committees focused on service enhancement.

Qualifications
