Dusk To Dawn, LLC is seeking experienced Registered Nurses (RN’s) and Social Workers with strong professional relationships throughout the healthcare community to serve as Patient Lead/Referral Liaisons.

This position is ideal for a healthcare professional who understands discharge planning and care transitions and has established connections with hospitals, rehabilitation facilities, skilled nursing facilities, assisted living communities, physician offices, case managers, and other healthcare providers.

Responsibilities

Develop and maintain referral relationships with:

Hospital discharge planners

Social workers and case managers

Rehabilitation centers

Skilled nursing facilities

Assisted living communities

Physician offices and specialty practices

Other healthcare organizations

Educate referral sources regarding Dusk To Dawn, LLC home care services.

Identify prospective patients requiring home healthcare or personal care services.

Coordinate referrals with the agency's intake/admissions team.

Maintain ongoing communication with referral partners to support continuity of care.

Represent the agency professionally within the healthcare community.


Qualification

Current RN license or degree in Social Work (preferred).

Existing professional network within the Baltimore and Washington, DC metropolitan healthcare market.

Knowledge of hospital discharge planning and post-acute care.

Excellent communication, relationship-building, and marketing skills.

Self-motivated with the ability to work independently.


Compensation

Base Pay: 10 hours weekly at $15.00 per hour

Commission: 15%–20% of the first month's patient service fees for each qualified patient referral that is admitted and begins services, based on experience, performance, and

referral volume.
