Accountabilities
  • The Population Health Pharmacy Technician is responsible for assisting in the coordination, implementation, and monitoring of pharmaceutical services aimed at improving health outcomes within a specific population or community. They collaborate with healthcare professionals, patients, and stakeholders to ensure effective medication management and adherence.

Role and Responsibilities:
  • Support medication reconciliation processes for patients within the designated population.
  • Collaborate with pharmacists and healthcare providers to optimize medication regimens.
  • Review patient profiles to identify potential medication-related issues and discrepancies.
  • Collect and analyze medication-related data to identify trends and opportunities for improvement.
  • Generate reports on medication adherence, therapeutic outcomes, and other relevant metrics.
  • Work closely with healthcare teams to ensure seamless coordination of pharmaceutical services.
  • Communicate with patients, caregivers, and healthcare professionals to address medication-related concerns.
  • Participate in population health programs aimed at promoting medication adherence and preventive care.
  • Support initiatives focused on improving health outcomes and reducing medication-related disparities.
  • Ensure compliance with regulatory standards, pharmacy laws, and organizational policies.
  • Contribute to quality improvement efforts within the pharmacy setting.

Requirements
Minimum Qualifications and Requirements:
  • High school diploma or equivalent; completion of a pharmacy technician program preferred.
  • Active licensure as a pharmacy technician in Tx and state of residence .
  • Strong understanding of pharmacy operations, medication management, and healthcare systems.
  • Excellent communication, interpersonal, and organizational skills.
  • Proficiency in using pharmacy software and computer systems for data entry and analysis.
  • Ability to work collaboratively in a multidisciplinary team environment.
  • Fluent in Spanish (preferred).
