Position Overview

Well Spring Healthcare Services is seeking a dedicated and compassionate Licensed Independent Clinical Social Worker (LICSW) to join our team in a full-time, remote capacity. In this role, you will be instrumental in providing high-quality psychosocial support to senior clients navigating the complexities of aging, mental health challenges, and life transitions. Your expertise will assist individuals in regaining control over their emotional well-being, specifically focusing on managing anxiety and depression.

Work Environment

This position is primarily remote, allowing for flexibility while ensuring that senior clients receive consistent care. Our work setting involves collaboration with team members including nurses, caregivers, and other healthcare specialists, all while maintaining a supportive and respectful dialogue to foster a culture of teamwork and compassion. The designated work hours are Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend availability based on client needs.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to understand the emotional, mental, and social factors impacting seniors and their families.
  • Collaborate directly with clients and their families to design and implement tailored care plans that align with their specific goals and preferences.
  • Provide counseling and emotional support, assisting seniors and their families as they navigate life transitions, chronic illnesses, or end-of-life decisions.
  • Advocate for clientsโ€™ rights and secure access to necessary community resources and services, enhancing their support network.
  • Partner with interdisciplinary teams, including healthcare professionals and social workers, to coordinate care that promotes clients' overall well-being.
  • Regularly visit and monitor clients to assess their living conditions, well-being, and to promptly address any safety concerns.
  • Facilitate communication and conflict resolution among family members involved in the care of senior clients, ensuring that everyone is on the same page.
  • Educate clients and their families about available community resources, programs, and support groups, encouraging greater social engagement.
  • Maintain meticulous and up-to-date documentation of client interactions, progress notes, and care plans in compliance with ethical and legal standards.
Patient Population / Client Focus

Your primary focus will be on senior clients, many of whom are facing challenges related to aging, chronic illness, and mental health issues. Understanding these unique circumstances allows you to provide targeted support that empowers them to improve their quality of life.

Qualifications
  • A Masterโ€™s Degree in Social Work from an accredited institution is required.
  • Current and valid state licensure as an Independent Clinical Social Worker (LICSW) is essential.
  • A minimum of two years of experience in a social work role, particularly in geriatric or community-centered settings, is expected.
  • Solid understanding of social work principles and aging-related challenges, along with familiarity with local community resources, is crucial for success in this role.
  • Strong assessment, counseling, and problem-solving skills to effectively support seniors.
  • Exceptional empathy, compassion, and the ability to build rapport with clients and their families.
  • Effective communication and interpersonal skills for collaboration with multidisciplinary teams.
  • Proficiency with electronic health record (EHR) systems and related software applications is preferred.
Preferred Experience

While a minimum of two years of social work experience is required, preference will be given to candidates with a background working specifically with seniors, particularly in hospice or palliative care environments. Familiarity with end-of-life issues and grief counseling would be advantageous.
