Position Summary

UPLIFT HOMECARE, LLC is seeking a compassionate, dependable, and skilled Licensed Physical Therapist Assistant (PTA) to join our growing home health team serving Lutz and surrounding communities.

This position provides an opportunity to deliver meaningful one-on-one patient care in the home while working alongside a supportive clinical team dedicated to helping patients improve mobility, function, safety, and independence.

The PTA will provide skilled therapy services under the supervision of a licensed Physical Therapist and in accordance with the patient's plan of care, physician orders, and agency policies.

Responsibilities

As a Physical Therapist Assistant, you will:

  • Implement physical therapy services according to the treatment plan established by the supervising Physical Therapist and physician.
  • Provide therapeutic exercises, treatments, and interventions designed to improve mobility, strength, balance, coordination, function, and independence.
  • Assist patients in achieving functional goals established by the Physical Therapist.
  • Observe, document, and report patient responses to treatment and changes in condition to the supervising PT and Clinical Manager.
  • Assist with patient functional assessments and communicate clinical findings to the supervising Physical Therapist.
  • Complete accurate, timely, and compliant clinical documentation and progress reports.
  • Educate patients and caregivers regarding exercises, mobility, fall prevention, safety, and treatment goals.
  • Participate in care coordination, interdisciplinary communication, and case conferences as appropriate.
  • Promote patient safety and independence in the home environment.
  • Participate in quality improvement initiatives and staff in-service education.
  • Maintain current knowledge of physical therapy practices, standards, and documentation requirements.

Qualifications

  • Current Florida Physical Therapist Assistant (PTA) license.
  • Graduate of an accredited Physical Therapist Assistant program.
  • Successful completion of the national PTA examination.
  • Minimum 1 year of home health experience required.
  • Current CPR certification.
  • Strong communication, organization, and documentation skills.
  • Ability to work independently while collaborating effectively with the clinical team.
  • Valid driver's license.
  • Reliable transportation.
  • Current auto insurance.
  • Ability to travel to patient homes throughout Lutz and surrounding areas.
